2.Anxiety Level, Disease Adaptation and Integrative Treatment Applications in Hypertension Patients

INTRODUCTION: This research was conducted to examine the epidemic anxiety level, adaptation to the illness, and preference for integrative treatment of hypertension patients during the Covid-19 pandemic.
METHODS: The research is descriptive and cross-sectional. In the research, Personal Information Form, Epidemic Anxiety Scale, Adaptation to Chronic Illness Scale (ACIS), Turkish Modified Morisky Scale (MMS) were used.
RESULTS: In analyses conducted with 300 hypertension patients, it was determined that 71.7% had knowledge about integrative practices, and 39.7% used thyme tea, cupping, lemon juice, garlic, ginger, olive leaf tea, and pomegranate syrup for hypertension. It was found that patients using integrative methods had higher epidemic illness anxiety levels than patients who did not use integrative methods. The patients’ knowledge and motivation levels regarding medication use habits were found to be high, and their illness compliance levels were found to be above the average value.
DISCUSSION AND CONCLUSION: During the pandemic period, patients’ anxiety levels were found to be high due to reasons such as social isolation and reduced face-to-face medical contact, and this may have increased patients’ tendency towards integrative treatment methods.

3.Investigation of Unplanned Out-of-Hospital Births

INTRODUCTION: The aim of this study was to retrospectively examine the demographic characteristics, reaction times, and outcomes of out-of-hospital birth cases received by the Ankara 112 Emergency Health Services Emergency Health Automation System and transported to Etlik City Hospital. The study aims to help improve the prehospital health services provided.
METHODS: In this study, out-of-hospital delivery calls made to the Ankara 112 Emergency Health Services Emergency Health Automation System (ASOS) between October 1, 2022, and June 1, 2024, and delivery cases brought to the Ankara Etlik City Hospital Gynecology Clinic were examined. A total of 49 cases were included. Data were analyzed using IBM SPSS 27.0 software; descriptive statistical methods and various comparison tests were applied.
RESULTS: The mean age of the cases included in the study was 27.4±5.7 years. The majority of the cases (71.4%) occurred outside of working hours. The nationality of 23 cases was the Republic of Türkiye. Patients with other nationalities had lower hemoglobin values. The mean hemoglobin value of the cases was 11.2±1.5. Forty-two cases were transferred to hospitals, and seven of them requested transfer between hospitals.
DISCUSSION AND CONCLUSION: This study shows that Ankara 112 Emergency Health Services intervenes quickly and effectively in prehospital delivery cases. A difference was found between patient nationalities in terms of hemoglobin values. Although the time to reach the case in prehospital health services is below the quality standards, further shortening of these times will have a positive impact on patient outcomes. Expanding such studies will improve the quality of prehospital emergency health services.

4.Radiologic Evaluation of Alternative Sites for Successful Needle Decompression of Pneumothorax

INTRODUCTION: The aim of this study was to evaluate chest wall thickness (CWT) using computed tomography (CT) and to determine which anatomic region should be selected for a successful needle thoracostomy (NT) procedure site in patients with a pneumothorax (PTX).
METHODS: The 147 patients who had thorax CT in the emergency department (ED) were included. The CWTs at the 2nd intercostal space (ICS) midclavicular line (MCL), at the 5th ICS midaxillary line (MAL), and the anterior axillary line (AAL) were measured on both sides.
RESULTS: The median CWTs were 27.2 mm and 27.3 mm on the right and left sides of the 2nd ICS-MCL, respectively. The median CWTs were 43.5 mm on the right and 43.8 mm on the left side of the 5th ICS-MAL. The median CWTs were 32.2 mm on the right and 31.2 mm on the left 5th ICS-AAL. This study also showed that when a standard 50-mm catheter was used, NT failed in 12.9% of patients in the 2nd ICS-MCL, in 67.4% of patients in the 5th ICS-MAL, and in 27.2% of patients in the 5th ICS-AAL.
DISCUSSION AND CONCLUSION: Our study confirmed that a 50-mm catheter, which is used classically in tension pneumothorax cases, has higher success rates than the 5th ICS when applied from the 2nd ICS. It should be noted that the thickness of the thorax wall increases in proportion to body mass index, which may lead to a failure of the procedure.

5.The Impact of Urinary Incontinence on Female Sexual Function: A Tertiary Center Experience

INTRODUCTION: Urinary incontinence (UI) is a prevalent health issue affecting women’s quality of life and sexual function. This study investigates the impact of different types of UI on sexual function using the Pelvic Organ Prolapse/Urinary Incontinence Sexual Function Questionnaire short form (PISQ-12).
METHODS: A total of 123 sexually active women with stress, urge, or mixed-type UI, who presented to the Urogynecology clinic of Etlik City Hospital between May 2023 and September 2023, were included in the study. Patients with a history of pelvic floor surgery or pelvic organ prolapse were excluded. The PISQ-12 short form, validated in Turkish, was administered. Statistical analysis was performed using SPSS 21 software.
RESULTS: The study population had a mean age of 48.2±10.5 years and a mean BMI of 30.0±4.6 kg/m². The average duration of UI was 5.4±3.8 years. Mixed-type UI was the most common (46.3%), followed by stress UI (36.5%) and urge UI (17.2%). The mean total PISQ-12 score was 27.1±6.7, with the lowest scores observed in the mixed-type UI group (25.1). Coital incontinence was reported in 46% of the patients. Statistically significant differences were observed in coital incontinence and coital incontinence anxiety between groups with poor and better sexual function (p<0.05). However, there were no statistically significant differences in PISQ-12 scores between the types of UI. Additionally, no significant difference in sexual dysfunction was found between premenopausal and postmenopausal groups.
DISCUSSION AND CONCLUSION: Urinary incontinence significantly impacts women’s physical, psychosocial, and sexual health, necessitating thorough evaluation and treatment to improve quality of life and sexual function. Addressing UI and its associated sexual dysfunctions in clinical practice is crucial, despite the challenges in diagnosing these conditions.

6.Wilsons Disease: Brain Magnetic Resonance Imaging

Wilson’s disease (WD) is an autosomal recessive systemic disorder that causes copper accumulation and toxicity in multiple organs. Various neurologic symptoms, such as rigidity, tremor, bradykinesia, dystonia, chorea, dysarthria, and dysphagia, are observed in the central nervous system involvement of WD. Magnetic Resonance Imaging (MRI) usually shows increased T2-weighted signals in the basal ganglia, mesencephalon, and pons. MRI is a valuable, non-invasive imaging technique for the diagnosis and follow-up of central nervous system involvement in life-threatening WD. In this case report, we will present cranial MRI findings of WD with cranial involvement.

7.Peroneal Nerve Entrapment Neuropathy in a Patient With Right Hemiplegia Due to Posttraumatic Subdural Hematoma

Peroneal nerve injury develops as a result of trauma or mechanical or postural compression of the nerve at the fibular head. It frequently occurs due to trauma; however, tumor, intraneural ganglion, hematoma, cyst, or iatrogenic causes also play a role in its etiology. In the differential diagnosis of peroneal nerve involvement, which can lead to weakness in foot dorsiflexion, L5 radiculopathy, lumbosacral plexus lesion, partial lesions of the sciatic nerve, and motor neuron disease should be considered. In peroneal nerve entrapment neuropathy, the inversion of the foot is normal because
the tibialis posterior muscle, which provides inversion of the foot, is innervated by the tibial nerve. In the differential diagnosis, examination and imaging methods such as direct radiographs, EMG, and MRI should be used in addition to clinical examination. In this article, we present peroneal entrapment neuropathy detected simultaneously in a right hemiplegic patient with a diagnosis of subdural hematoma after a vehicular traffic accident. In immobilized patients with loss of consciousness, prolonged intubation, and intensive care unit hospitalization, entrapment of peripheral nerves due
to external compression may be observed. When planning our diagnostic and treatment algorithm, personal habits and possible etiologic factors should be evaluated together.

8.Acute COVID-19 Case with Myocardial Involvement in Adolescent with Two-Dose mRNA Vaccination

COVID-19 infection usually causes mild symptoms in children. Mortality and morbidity of the disease have decreased with the widespread use of vaccines. COVID-19 vaccines are generally known to protect against the disease and its complications. In this case report, a thirteen-year-old patient who came to the pediatric emergency department with chest pain, received two doses of BNT162b2 (Pfizer-Biontech), and was diagnosed with acute COVID-19-associated myocarditis is presented. With this case, it was emphasized that it may be necessary to make vaccination reminder doses at shorter intervals in order to evaluate those with chronic diseases separately from healthy children and to maintain effective protection against COVID-19.
